Step 1: You must read and initial the Vendor Letter below.

Please read completely and initial in the box - where directed, before proceeding to the MN ST19 form in Step 2.

Step 2: You must fill out the MN ST19 form completely before you can proceed to the Vendor Registration Form in Step 3.

Step 3: Fill out the Vendor Registration Form, choose your preferences and checkout via credit card. You will receive an email receipt following a successful transaction. 

These documents and your payment information will be sent together to the event organizers. Thank you!
